The first screen of the proof of service is pretty large and involved, but once you fill it in properly, your affidavit or proof of service can be pretty simple.
Please note the first selection (drop down list at the top), it is the manner of service. You have to fill that in to start your proof.
Note the date and time just below. Set those values to the date and time of service. If you don't check the box they will be blank.
if you are doing a subserve or business serve the party served will open up so you enter the person's name and relationship.
Please review the address of service here and make any changes that are needed:
If they refuse to give their name fill in the physical description portion of this page.
If you dispatched or assigned the job to a server, the system will default that server to this page.
If not the none assigned server will show on the server drop down list. Please pick the correct server. it is a lot easier to get it right here than to update the proof later.
if this service requires a mailing, check the box and select the mailing employee along with the mailed from information and set the date of mailing.
Finally, if you want to include diligence on the proof or a separate page, check the box for diligence required.
If you state requires notary, check the date signed and select your notary to pull that information on your affidavit.
The fee advance, weight, pieces, distance and wait time fields are important if you have billing items that use that information. If not, you can ignore this part of the page.
Once you leave this page, you can't enter that information easily, so update it here if needed.
Once you complete entry on the above screen the system will show you the tabbed proof screen:
You can update each tab as needed. You may enter additional diligence as needed on the diligence tab.
The files tab is used to upload images to the proof.
The report tab is used as a final status on you affidavit.
If you have additional mailing information and want additional mailing affidavits, enter the information on the additional mailing tab.
Each state has its own proof and affidavit requirements. If our standard forms don't work in your state, then please post a ticket as needed.
